Schedule a day and time to talk with a board-certified genetic counselor using our online appointment schedule.

If you cannot access our online appointment schedule for any reason, you can call us at 1.800.975.4819 and we will help you schedule an appointment by telephone.

Enter your family medical history through our online family medical history questionnaire.

If you cannot access our online family medical history questionnaire for any reason, you can call us at 1.800.975.4819 and we will collect your family medical history by telephone.

It is important that we have your family medical history (either through our online tool or by telephone) at least 48 hours prior to your scheduled appointment. This allows your board-certified genetic counselor time to review your family history in preparation for your counseling session.

On the day and time of your appointment, your board- certified genetic counselor will call you at the telephone number you provided when you scheduled your appointment.
